    What is Community Health?

    What We Do

    Community health emphasizes non-clinical strategies to enhance the well-being of populations through health education and promotion, with a key emphasis on collaboration with community partners to effectively address health needs and disparities.


    SCHD's dedicated Community Health Heam is staffed with a Public Health Nurse and Community Health Specialist. They create resourceful health programs that promote the overall well-being and prioritize the health needs of the community. 

    Social Determinants of Health (SDoH)

    Social determinants of health encompass the social and economic conditions in which people are born, grow, live, work, and age. These conditions influence communities access to resources and opportunities that impact health outcomes. In community health, addressing social determinants involves implementing comprehensive strategies that tackle the root causes of health disparities.

     Every individual and community organization possesses the ability to influence the overall health and wellness of their towns. Whether attending health education programs and trainings or partnering with SCHD, collaboration with community members stands as a cornerstone for success. Prioritizing the ethos of "for the community and by the community" remains paramount  to create lasting impact and cultivate healthier, happier communities.
